Subject: Request for Change of Athlete’s Surname in Association Records

Sir/Madam,

I, Kakani Vijaya, am writing this letter to request a correction/change in the surname of the athlete KAKANI VIJAYA, who is registered under the association with the registration number ID No. NT-9841

Due to error during registration, the correct surname should be “KAKANI” instead of “KAKNI”.

I kindly request you to update the association’s records, certificates, and any official documents accordingly. Enclosed herewith are the supporting documents for your reference is Copy of Aadhaar Card along with full application attached

I would be grateful if the correction could be made at the earliest. Please acknowledge the receipt of this request.
